This form is for students who have already completed the Application for Admission. If you currently hold an F‑1 visa or are in the process of applying for one, you must submit the required supporting documents to complete the admission process.

Medical Insurance

All F‑1 students and their dependents must have medical insurance before starting at POLY. You may choose your own plan, or you may enroll in our recommended insurance plan, the ISP plan offered through J. Deutsch Associates, Inc.

When enrolling through ISP, select “Poly Languages Institute” from the School/Organization drop‑down menu to view the available plan options.
